A shocking new investigation is exposing what may be one of the largest examples of government waste in America today. Billions of taxpayer dollars intended for Medicaid services are allegedly flowing into loosely monitored home health businesses, raising serious concerns about fraud, abuse, and the stewardship of public funds.

Just before the Department of Government Efficiency closed its doors, it made a quiet move that may end up as its most lasting impact on the federal deficit.
DOGE published a massive trove of data in February that, for the first time, lets the public see what companies are billing Medicaid for. For decades, the payouts have been shrouded in secrecy. One of the largest government programs was a black box.
I’ve spent the past two months diving into the numbers. What I found was the most blatant waste of federal dollars that I have encountered in my two decades as an investigative reporter.
The investigation focuses heavily on Ohio, where Medicaid waivers have dramatically expanded home health and “personal services” programs. According to the report, billions of dollars are being spent on services such as companionship, homemaking, cooking, and cleaning — often performed not by trained healthcare workers, but by family members of Medicaid recipients themselves.
Investigators found entire office complexes in Columbus packed with home health companies that appear nearly identical, many with little visible activity despite billing taxpayers millions of dollars. One building alone reportedly housed 94 Medicaid-billing companies and generated $66 million in taxpayer-funded payments over several years.
The report also uncovered alarming patterns among some company owners, including unpaid taxes, criminal convictions, fraudulent business histories, and rapidly created LLCs. Several businesses allegedly transformed overnight from unrelated industries — such as janitorial services — into Medicaid-funded healthcare providers generating enormous revenues.
Perhaps most troubling is the apparent lack of oversight. Since many services occur inside private homes, there is often little way to verify whether the billed services actually occurred. In many cases, individuals are effectively paid by the government to spend time with their own relatives, with minimal accountability or verification.
As intercessors, let’s pray for wisdom, integrity, and accountability within every level of government. Taxpayer resources are meant to serve the truly vulnerable — not enrich dishonest actors or enable corruption. At a time when America faces staggering national debt and economic uncertainty, this investigation highlights the urgent need for righteous stewardship and honest leadership.
